may be used and disclosed and how you can access this information. We are required by law to
protect the privacy of your PHI, provide this Notice of our legal duties and privacy practices, and
comply with the terms of this Notice.
Uses and Disclosures of PHI

We may use and disclose PHI for treatment, payment, and health care operations, and for other
purposes permitted or required by law. Uses or disclosures not expressly permitted by law or
this Notice will be made only with your specific written authorization, which you may revoke in
writing at any time, except where action has already been taken in reliance on it.

Your Rights

You have rights under HIPAA, including the right to access and obtain copies of your health
records, request amendments, request restrictions or confidential communications, receive an
accounting of disclosures, obtain a paper copy of this Notice, and file a complaint if you believe
your privacy rights have been violated. These rights are subject to certain legal limitations.
Special Protections for Substance Use Disorder Records (42 CFR Part 2)
Records related to the diagnosis, treatment, or referral for treatment of a substance use
disorder are protected by federal law under 42 CFR Part 2. Even if our practice does not provide
substance use disorder treatment, we may receive Part 2–protected records as part of your
health history. When received or maintained by our practice, these records are subject to
heightened confidentiality protections and may not be disclosed or re-disclosed except as
permitted by federal law or with your specific written authorization.
